My hooks aren't working / I see "Duplicate hooks file" errors This is the most common issue. **Do NOT add a `"hooks"` field to `.claude-plugin/plugin.json`.** Claude Code v2.1+ automatically loads `hooks/hooks.json` from installed plugins.
